We are hiring on behalf of a leading NHS partner organisation who are expanding their Continuing Healthcare team. They require an Interim CHC Nurse Assessor with specialist experience in Learning Disability. This role is hybrid and sits outside of IR35, with an immediate start preferred.

Duties:

• Carry out Continuing Healthcare assessments including DSTs for individuals with complex Learning Disability needs
• Work collaboratively with multidisciplinary teams to gather clinical evidence
• Review care packages and make recommendations regarding CHC or Funded Nursing Care eligibility
• Engage with families and advocates to ensure a clear understanding of the CHC process
• Complete accurate clinical documentation and contribute to high quality decision making

Essential criteria:

• Registered Nurse with proven experience in Learning Disability or complex needs
• Strong understanding of the National Framework for NHS Continuing Healthcare
• Excellent assessment skills with the ability to analyse complex clinical information
• Confident communication and stakeholder engagement across health and social care
